Definitions

We use common language and definitions to help you understand important terms throughout the Privacy Statement and Terms of Use.

“Endeavor Health”, "EH", “We”, “Our” or “Us” — Endeavor Health includes our hospitals (Edward Hospital, Elmhurst Hospital, Evanston Hospital, Glenbrook Hospital, Highland Park Hospital, Linden Oaks Hospital, Northwest Community Hospital, Skokie Hospital and Swedish Hospital), our subsidiaries and affiliates.
“Endeavor Health Services” — Collectively refers to all of the following services that are owned by, operated and/or managed by or for Endeavor Health:
- “Endeavor Health Websites” — Our websites include, but are not limited to, endeavorhealth.org, northshore.org, eehealth.org, nch.org and swedishcovenant.org.
- “Endeavor Health Patient Portals” — Our patient portals allow patients to access their medical records, schedule visits and more using a web browser or mobile app. Our patient portals include, but are not limited to:
  - NorthShoreConnect via web, Apple iOS and Google Android
  - Edward-Elmhurst Health MyChart via web, Apple iOS and Google Android
  - NCH MyChart via web, Apple iOS and Google Android
- “Other EH Mobile Apps” — Separate from our patient portal’s mobile apps, we manage and offer additional mobile apps for our patients, team members and communities.
- “Endeavor Health Additional Services” — We also provide services, digital offerings and tools, including, but not limited to: customer service; technical support; QR codes; chatbots; search tools; clinician profiles; location listings; digital signage; and subscriptions, reminders, alerts and marketing messages using direct mail, email and SMS (text messages).
“You” or “your” — You, regardless of being an individual, a group or an organization, and regardless of whether you are using Endeavor Health Services on behalf of yourself or someone else.
“Your information” — Collectively refers to an individual’s personally identifiable information (“PII”), non-personally identifiable information (“PII”) and/or protected health information (“PHI”).
“Terms” — Refers to this Endeavor Health Terms of Use.
“Content” — Refers to text, marks, information, images, photos, illustrations, data sets, graphics, user interfaces, artworks, logos, videos, audios, infographics, directories, listings, databases, and search engines. Includes downloadable content, such as PDFs, audio and video files.
Protected health information (“PHI”) — Any information regarding an individual's medical history, mental or physical condition, or medical treatment or diagnosis by a healthcare professional, including such information provided to a website or mobile application.

Please review our Terms as they contain certain obligations and responsibilities that may affect your legal rights and ability to use the Endeavor Health Services. For more information regarding how we collect and process personal information in connection with your use of the Endeavor Health Sites, please review our Privacy Statement.

While committed to providing you information about the services and treatment Endeavor Health provides, we provide the information via Endeavor Health Services as a courtesy.By providing information via Endeavor Health Services, Endeavor Health is not attempting to practice medicine, and use of the Endeavor Health Services does not, by itself, establish a physician-patient relationship. The Endeavor Health Services are designed for informational and educational purposes only. It should not be construed as medical advice nor used for diagnostic purposes. We do not intend the Endeavor Health Services to be a substitute for medical advice specific to your situation. If you feel you have a health problem, please consult a medical professional.

By accessing the Endeavor Health Services or using any of the features or services provided on the Endeavor Health Services, you agree and consent to comply with and be bound by these Terms. You agree that you will not violate any local, state, federal or international laws in using the Endeavor Health Services or accessing any material on the Endeavor Health Services. If you violate any of our Terms, Endeavor Health reserves the right to prohibit, restrict or discontinue your access to certain or entire features within the Endeavor Health Services. Endeavor Health also reserves the right to discontinue any of the Endeavor Health Services either temporarily or permanently or discontinue any functionality on the Endeavor Health Services at any time and without notice. Endeavor Health may modify these Terms at any time without notice. If you do not agree to the Terms and the Privacy Statement, please do not use or access the Endeavor Health Services.

Special terms may apply to some services offered by the Endeavor Health Services, or to any features, promotions or activities that may be offered by the Endeavor Health Services (the “Activities”). Such special terms (which may include official rules and expiration dates) may be posted in connection with the applicable service or Activity. By entering or participating in an Activity you will become subject to those terms or rules. We urge you to read the applicable terms or rules, which are linked from the particular Activity, and to review our Privacy Statement which, in addition to these Terms, governs any information you submit in connection with such Activity. Any such special terms or rules are in addition to these Terms and, in the event of a conflict, any such terms or rules shall prevail over these Terms.

While Endeavor Health believes in providing you with accurate information, Endeavor Health can only provide general information about Endeavor Health’s services and facilities. In other words, the information provided on the Endeavor Health Services may not be cumulative or specific to your situation. Please do not delay or forgo seeking medical care from a physician or other qualified healthcare provider if you have questions, concerns or symptoms related to the information provided via Endeavor Health Services.

Accounts, passwords and security

You are responsible for taking the necessary steps to ensure that no other person has access to your password or account. It is your sole responsibility to (1) control the dissemination and use of your account details, such as log usernames and passwords; (2) authorize, monitor and control access to and use of your password; (3) ensure your account details are kept updated and accurate; and (4) promptly inform Endeavor Health of any need to deactivate an account.

If your account or information has been compromised, please contact us immediately to report this incident.

Endeavor Health's use of your personal information and your responsibilities in connection with privacy is in accordance with our Privacy Statement and is incorporated into by reference into these Terms of Use.

Privacy

Endeavor Health is committed to protecting the individual privacy of our patients, team members and visitors. Please review our Privacy Statement to understand how Endeavor Health uses your personal information. You are also encouraged to review the Notice of Privacy Practices to understand how we collect and use your protected health information.

Eligible users of Endeavor Health Services

Endeavor Health is committed to protecting the privacy of children and complies with the Children’s Online Privacy Protection Act. As such, the Endeavor Health Services are not intended for or directed to children under the age of 13. If you are under the age of 18 (or under the age of majority set by the applicable laws of the State in which you are located), please do not use the Endeavor Health Services without being supervised by a parent or legal guardian who agrees to be bound by these Terms. By using the Endeavor Health Services, or by supervising someone using the Endeavor Health Site, you represent that you are at least 18 years old and have the full power and authority to enter into these Terms. If you are accessing or using the Endeavor Health Services on behalf of another individual(s), you represent that you are authorized to accept these Terms on that person’s or entity’s behalf, and that person or entity agrees to be responsible to Us for your violation of these Terms.

License grant

Endeavor Health grants you a personal, revocable, limited, non-exclusive, non-transferable license to access and use the Endeavor Health Services, provided you accept and comply with these Terms. You may use the Endeavor Health Services for your non-commercial personal or educational use. Please do not use the Endeavor Health Services for any other purpose.

License restrictions

Unless you receive Our prior written consent, you may not modify, translate, create derivative works of, copy, distribute, market, display, remove or alter any proprietary notices or labels from, lease, sell, sublicense, clone, transfer, decompile, reverse engineer, or incorporate into any information retrieval system (electronic or mechanical) any portion of the Endeavor Health Services.

Please do not access or use the Endeavor Health Services for any unauthorized or illegal purpose or activity, interfere with the proper working of the Site or interfere with any other person’s use and enjoyment of the Endeavor Health Services.

If you are unable to comply with these restrictions, we may restrict your use of the Endeavor Health Services, such as by suspending or terminating your ability to access all or part of the Endeavor Health Services.

Intellectual property rights & license

Endeavor Health owns or licenses all the Endeavor Health Services’ original content, materials, features and functionality (including, without limitation, text, marks, information, images, photos, graphics, artworks, logos, videos, audios, directories, listings, databases, and search engines) (the “Content”). Please be advised that the Content may be protected by copyright, trademark, or other intellectual property laws. For as long as you comply with these Terms, Endeavor Health grants you a limited, non-exclusive, non-transferable license (“Content License”) to use the Content solely for your non-commercial personal or educational use. The Content License does not include a right to: (1) sell or resell the Content; (2) copy or reproduce the Content for any purpose other than your non-commercial personal or educational use; (3) modify the Content; (4) remove any proprietary rights notices or markings, or otherwise make a derivative use of the Content; (5) use any data mining, robots or similar data gathering or extraction methods; and/or (6) use the Content other than for its intended purposes. Except for the express, limited Content License set forth in this paragraph, We reserve all other rights in and to the Content.

If you cannot comply with the limitations of the Content License, Endeavor Health may revoke or terminate your Content License. Any unauthorized use, reproduction or distribution of the Content is strictly prohibited and may result in termination of the Content License, as well as potential civil and/or criminal penalties.

Sharing content

You may download, copy or otherwise use the Content and other downloadable items displayed on the Endeavor Health Services (“Downloadable Content”) for non-commercial personal or educational uses. If you download, copy or otherwise use Downloadable Content, you are required to maintain all copyright, trademark and/or other notices contained therein. Copying or storing of any Content for purposes other than non-commercial personal or educational use is expressly prohibited without prior written permission from Endeavor Health. We may require you to stop using Downloadable Content should your use violate Endeavor Health’s policies, rights or cause interruption or deterioration of the Endeavor Health Services or Content.

Submissions

You are solely responsible for the information that you post, share, email, transmit or otherwise make available via the Endeavor Health Services (“Submission”). All Submissions are subject to these Terms. Endeavor Health is under no obligation to post or use any Submission and may remove any Submission at any time in its sole discretion.

By making a Submission, you represent and warrant that your Submission is true, accurate and not misleading; that you own all right, title and interest, including copyright, to your Submission, and that your Submission is your own original work; that your Submission does not include the trade secret or confidential information of others, and does not infringe any other person’s or entity’s rights; and that you and any other person or entity (or its owner) mentioned or shown in your Submission hereby release and hold harmless Endeavor Health and its designees from and against any and all claims concerning Endeavor Health’s or its designees’ use, modification or distribution of the Submission or any part thereof. You must hold all necessary releases, licenses and consents concerning the contents of your Submission and rights to post, distribute, or transmit your Submission.

You agree that you will evaluate and bear all risks associated with your Submission and any disclosure in your Submission. Submissions will be available to Endeavor Health Site users and to users of other websites and services.

By making a Submission, you grant Endeavor Health and its affiliates, licensees, assignees and designees an irrevocable, assignable, transferable, fully sub-licensable (through multiple levels of sublicensees), perpetual, world-wide, royalty-free, fully paid-up, non-exclusive license, in their sole discretion, to use, distribute, reproduce, modify, combine, adapt, publish, translate, rent, lease, sell, publicly perform, publicly display and create derivative works of your Submission (in whole or in part), along with your name or any part thereof, your city/town/village of residency, and other information from your user profile, in Endeavor Health’s sole discretion, via the Endeavor Health Services or elsewhere, and to use or incorporate all or any part of your Submission into other advertising, promotion, marketing, review, recommendation, research, analysis or other materials in any format or medium now known or later developed. You hereby waive any right to inspect such use and waive, release, and hold Endeavor Health and its affiliates, licensees, assignees and designees harmless against any and all claims based on privacy, publicity, defamation, misappropriation, intellectual property or similar claims for any use of your Submission.

Text Messaging and Marketing Communications

Where you have provided us your mobile phone number and opted in to text messaging, you expressly consent that Endeavor Health its affiliates, agents and representatives may call or text you (including SMS and MMS text messages and push notifications) about your account, or to provide you with recurring special offers and marketing messages related to additional products, services, or promotions offered by Endeavor Health, through the use of an automatic telephone dialing system and/or an artificial or prerecorded voice. You further represent and warrant that you are the authorized subscriber to the telephone number(s) you have provided above and that you have the authority to provide the consent described above at the number(s). Message and data rates may apply. You are not required to give your consent as a condition of purchasing any products or services from Endeavor Health. To revoke consent to contact you by text (including SMS and MMS messages), you may respond “STOP” to any of our text messages. Links to the Endeavor Health Services

Endeavor Health grants you a limited, non-exclusive right to create a hyperlink to any portion of Endeavor Health Services for non-commercial personal or educational purposes. If you create a hyperlink to any portion of the Endeavor Health Services, we ask that you ensure such link does not portray Endeavor Health, including any Endeavor Health Endeavor Health Party (as defined below in the section titled “Limitation of liability”), or any Content in a false, misleading, derogatory or otherwise defamatory manner. We also ask that you ensure that the linking site does not contain any adult material or other material that is offensive, harassing or otherwise objectionable. If you are unable to comply with the limitations in this section, we may require you to disable or remove any hyperlink to the Endeavor Health Services that violates Endeavor Health’s policies (including these Terms), rights or causes interruption or deterioration of the Endeavor Health Services.

Use of the Endeavor Health Services

When accessing, browsing or using the Endeavor Health Services, please do so responsibly. Do not violate any local, state or federal law, rule or regulation. Also, do not violate any contract or third-party right, such as intellectual property or other proprietary rights. Unless otherwise expressly allowed by these Terms, the following actions or activities are expressly prohibited while accessing, browsing or using the Endeavor Health Services:

Engaging in any harassing, threatening, intimidating, predatory or stalking conduct;
Copying, reproducing, selling, leasing, renting, distributing, transferring, modifying or making derivative works of the Endeavor Health Services or Content;
Decompiling, disassembling, reverse engineering or otherwise attempting to discover or derive any proprietary software code or other information associated with the Endeavor Health Services or Content;
Removing or altering any notice or other legal indication of a copyright, trademark or other intellectual property right;
Interfering with the proper working of the Endeavor Health Services including, but not limited to, the transmission of any virus, worm, trap door, back door, timer, clock, Trojan horse, denial of service attack or other limiting routine, instruction or design;
Framing or otherwise creating a browser or border environment around any Endeavor Health Services page or Content, or “deep-linking” to any internal page or area of the Endeavor Health Services; and
Using the Endeavor Health Services in any way or for any purpose that violates international law.

If you cannot access, browse or use the Endeavor Health Services within the bounds of acceptable and responsible use of the Endeavor Health Services, as set forth in this section of these Terms, please do not access, browse or use the Endeavor Health Services. If you fail to adhere to the limitations of use set forth in this section of these Terms, Endeavor Health may revoke, suspend or terminate your ability to access, browse or use the Endeavor Health Services and/or any license or right granted by these Terms.

Information about our healthcare providers

At Endeavor Health, Our goal is to provide you with accurate information that lets you to make informed choices about your health. To this end, We strive to provide you complete information about physicians and other healthcare providers who practice medicine at Endeavor Health However, please understand that some physicians or other healthcare providers, whose information is available on the Endeavor Health Services, may not be employees or agents of Endeavor Health; we will make an attempt to state their affiliation with Endeavor Health. Although Endeavor Health believes the information (e.g. academic, professional credentials, etc…) provided on the Endeavor Health Services concerning its physicians or other healthcare providers is reliable and accurate, We cannot guarantee or warrant that such information is accurate, complete or up-to-date.

Links to third-party sites

For your convenience, the Endeavor Health Services may provide links (including any link from an online banner advertisement) to third-party sites on the Internet. Please note that Endeavor Health does not maintain such third-party sites and has no control over such sites. If you decide to access any third-party site via a link on the Endeavor Health Services, please understand that you do so entirely at your own risk. Also, please note that the appearance of a link to a third-party site is not necessarily intended as an endorsement of any particular company or product.

Endeavor Health disclaims all liability with regard to your access to and use of such information, material, products or services or transactions with such linked website or third parties. You acknowledge and agree that we shall not be responsible or liable, directly, or indirectly, for any damage, loss or other claim caused or alleged to be caused by or in connection with, access to, use of or reliance on any content available on or through any other site or resource.

YOUR CORRESPONDENCE AND BUSINESS DEALINGS WITH OTHERS FOUND ON OR THROUGH THE WEBSITE AND/OR SERVICES, INCLUDING, WITHOUT LIMITATION, THE PAYMENT AND DELIVERY OF SERVICES, AND ANY TERMS, CONDITIONS, WARRANTIES AND REPRESENTATIONS ASSOCIATED WITH ANY SUCH DEALINGS, ARE SOLELY BETWEEN YOU AND THE THIRD-PARTY. WE ENCOURAGE YOU TO REVIEW ALL POLICIES, RULES, TERMS AND REGULATIONS, INCLUDING THE PRIVACY POLICIES AND TERMS OF USE OF EACH AND ANY THIRD-PARTY SITE THAT YOU VISIT.

Click-through agreements

Before using certain areas of the Endeavor Health Services, you may be asked to indicate your acceptance of additional special terms by selecting a check-box or button marked “I Accept”, “I Agree”, “Okay”, “I Consent”, or other words or actions that similarly acknowledge your consent or acceptance of a Click-Through Agreement. To the extent there is a conflict between these Terms and any Click-Through Agreement for the activity in which you choose to participate, the Click-Through Agreement will govern.

Digital Millennium Copyright Act (“DMCA”) notice

Endeavor Health is committed to complying with intellectual property laws, and requires all users of the Endeavor Health Services to comply with these laws. Accordingly, you may not store any material or content, or use or disseminate any material or content, through the Endeavor Health Services in a manner that constitutes an infringement of a third party’s intellectual property rights, including rights granted by copyright law.

If you believe that work has been copied on the Endeavor Health Services in a way that constitutes copyright infringement, or that information on the Endeavor Health Services, including the Content, otherwise infringes an intellectual property right, please contact Endeavor Health in writing to Chief Legal Officer, 1301 Central St., Evanston, IL 60201.

When contacting Endeavor Health regarding a claim of copyright or other intellectual property right infringement, please provide the following:

an electronic or physical signature of the person authorized to act on behalf of the owner of the copyright or other intellectual property interest;
a description of the copyrighted work or other intellectual property that you claim has been infringed;
a description of where the material that you claim is infringing is located on the Endeavor Health Services;
your address, telephone number and email address;
a statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law; and
a statement by you, made under penalty of perjury, that the above information in your notice is accurate and that you are the copyright or intellectual property owner or authorized to act on the copyright or intellectual property owner’s behalf.

Applicable law

These Terms and all matters regarding your use of the Endeavor Health Services shall be governed by, construed in accordance with and enforced under the laws of the State of Illinois, without regard to choice of law principles. By accessing, browsing or using the Endeavor Health Services, you agree that any action at law or in equity arising out of or relating to these Terms shall be filed only in the state and federal courts located in or otherwise covering DuPage County, Illinois. You hereby irrevocably and unconditionally consent and submit to the exclusive jurisdiction of such courts over any suit, action, or proceeding arising out of these Terms.

Use of the Endeavor Health Services outside the United States

The Endeavor Health Services are intended for use by individuals located in the United States. If you are outside of the United States and access the Endeavor Health Services or submit your personal information to Us, please be advised that United States law may not offer you the same privacy protections as the law of your jurisdiction. If you visit the Endeavor Health Services or contact Us from outside the United States, please be advised that: (1) any information you provide to Us or that We automatically collect will be transferred to the United States; and (2) that by using the Endeavor Health Services or submitting information, you explicitly authorize its transfer to and subsequent processing in the United States in accordance with our Privacy Statement.

Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT WILL ENDEAVOR HEALTH (INCLUDING ENDEAVOR HEALTH’S AFFILIATES, SUBSIDIARIES, EMPLOYEES, OFFICERS, MEMBERS, OR TRUSTEES) NOR ANY OF ENDEAVOR HEALTH’S AGENTS, REPRESENTATIVES, SUPPLIERS, ADVERTISERS, CONTRACTORS, PROMOTIONAL PARTNERS, OR LICENSORS (EACH, A “ENDEAVOR HEALTH PARTY” AND COLLECTIVELY, THE “ENDEAVOR HEALTH PARTIES”) BE LIABLE FOR ANY INDIRECT, CONSEQUENTIAL, SPECIAL, INCIDENTAL OR PUNITIVE DAMAGES, HOWEVER CAUSED, REGARDLESS OF THE THEORY OF LIABILITY AND WHETHER OR NOT A ENDEAVOR HEALTH PARTY HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES, ARISING OUT OF OR RELATING TO: (1) THE USE OR INABILITY TO USE THE ENDEAVOR HEALTH SERVICES; (2) ANY CONTENT OR PRODUCTS AVAILABLE OR SOLD ON OR THROUGH THE ENDEAVOR HEALTH SERVICES; OR (3) ANY THIRD-PARTY CONTENT OR MATERIALS (INCLUDING USER CONTENT) OR THE DEFAMATORY, INFRINGING, OFFENSIVE, OR ILLEGAL CONDUCT OF ANY THIRD PARTY. WHILE CERTAIN JURISDICTIONS DO NOT ALLOW THE LIMITATION OF LIABILITY OF INCIDENTAL OR CONSEQUENTIAL DAMAGES, SUCH LIMITATION MAY NOT APPLY TO YOU. IN ANY CASE, THE ENTIRE LIABILITY OF THE ENDEAVOR HEALTH PARTIES UNDER THESE TERMS FOR ALL DAMAGES, LOSSES, AND CAUSES OF ACTION (WHETHER IN CONTRACT, TORT (INCLUDING NEGLIGENCE), OR OTHERWISE) IS LIMITED TO THE AMOUNT PAID BY YOU, IF ANY, FOR ACCESS TO THE ENDEAVOR HEALTH SERVICES.

In the event you are dissatisfied with, or dispute, these Terms, the Endeavor Health Services, and/or the Content, please terminate your use of the Endeavor Health Services, as Endeavor Health has no other obligation, liability or responsibility to you.

THE ENDEAVOR HEALTH SERVICES AND THE MATERIALS CONTAINED THEREIN, INCLUDING THE CONTENT, ARE PROVIDED ON AN "AS IS" OR “AS AVAILABLE” BASIS WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. ENDEAVOR HEALTH DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE AND NON-INFRINGEMENT. ENDEAVOR HEALTH DOES NOT REPRESENT OR WARRANT THAT MATERIALS ON ENDEAVOR HEALTH SERVICES ARE ACCURATE, COMPLETE, RELIABLE, CURRENT OR ERROR-FREE. ENDEAVOR HEALTH DOES NOT REPRESENT OR WARRANT THAT THE ENDEAVOR HEALTH SERVICES OR ITS SERVER(S) ARE FREE OF VIRUSES, WORMS, UNAUTHORIZED USERS OR HACKERS, “TROJAN HORSES” OR OTHER MALICIOUS SOFTWARE. ENDEAVOR HEALTH DOES NOT MAKE ANY WARRANTIES AS TO THE RESULTS THAT MAY BE OBTAINED FROM USE OF THE ENDEAVOR HEALTH SERVICES AND/OR SERVICES, OR ENDORSE, RECOMMEND OR MAKE ANY WARRANTIES AS TO THE ACCURACY, RELIABILITY OF ANY CONTENT, INFORMATION, MATERIALS, USER CONTENT, FEATURES, SERVICES, PRODUCTS OR STATEMENTS AVAILABLE ON OR THROUGH THE ENDEAVOR HEALTH SERVICES OR THROUGH LINKS ON THE ENDEAVOR HEALTH SERVICES. Indemnification

To the fullest extent permitted by law, you shall defend, indemnify and hold harmless the Endeavor Health Parties from and against all claims arising from or in any way related to your use of the Endeavor Health Services and/or the Content, your violation of these Terms or any other actions connected with your use of the Endeavor Health Services and/or the Content, including any liability or expense, losses, damages (actual and consequential), suits, judgments, litigation costs and attorneys’ fees. Endeavor Health will provide prompt written notice of any such claims, but failure to provide such notice will not release you from any of your obligations pursuant to this section, except to the extent that you are actually prejudiced by such failure, and will not relieve you from any other liability that You may have to the Endeavor Health Parties other than under this section.

Waiver

Endeavor Health’s failure to exercise or enforce any right or provision in these Terms shall not constitute a waiver of such right or provision.

Term and termination

These Terms will take effect at the time you begin using the Endeavor Health Services. Endeavor Health reserves the right, with or without notice, and at any time and for any reason, to deny you access to the Endeavor Health Services or to any portion thereof, and to terminate these Terms. These Terms, and the licenses and rights granted to you thereunder, will terminate automatically if you fail to comply with, or if We reasonably believe that you have not complied with, these Terms.

You may terminate our relationship under these Terms at any time by ceasing to access, browse, or use the Endeavor Health Services, but all applicable provisions, rights, and remedies available to Endeavor Health under these Terms will survive such termination. Upon termination, please destroy all copies of any portion of the Endeavor Health Services, including any Content, in your possession.

Severability

If a court of competent jurisdiction finds any provision of these Terms to be invalid, illegal, or unenforceable, such provision shall be enforced to the fullest extent of the law, and all other provisions shall remain in full force and effect.

Changes to these Terms

Endeavor Health reserves the right to revise these Terms at any time in Our sole discretion by posting revised Terms to the Endeavor Health Services. Because your use of the Endeavor Health Services signifies your acceptance of all the terms and conditions contained within the Terms in effect at the time of your use, We ask that you please regularly review the Terms posted to the Endeavor Health Services.

Definitions

We use common language and definitions to help you understand important terms throughout the Privacy Statement and Terms of Use.

“Endeavor Health”, “EH”, “We”, “Our” or “Us” — Endeavor Health includes our hospitals (Edward Hospital, Elmhurst Hospital, Evanston Hospital, Glenbrook Hospital, Highland Park Hospital, Linden Oaks Hospital, Northwest Community Hospital, Skokie Hospital and Swedish Hospital), our subsidiaries and affiliates.
“Endeavor Health Services” — Collectively refers to all of the following services that are owned by, operated and/or managed by or for Endeavor Health:
- “Endeavor Health Websites” — Our websites include, but are not limited to, endeavorhealth.org, northshore.org, eehealth.org, nch.org and swedishcovenant.org.
- “Endeavor Health Patient Portals” — Our patient portals allow patients to access their medical records, schedule visits and more using a web browser or mobile app. Our patient portals include, but are not limited to:
  - NorthShoreConnect via web, Apple iOS and Google Android
  - Edward-Elmhurst Health MyChart via web, Apple iOS and Google Android
  - NCH MyChart via web, Apple iOS and Google Android
- “Other EH Mobile Apps” — Separate from our patient portal’s mobile apps, Endeavor Health and/or other third parties’ may manage and/or offer additional mobile apps for our patients, team members and communities.
- “Endeavor Health Additional Services” — We also provide services, digital offerings and tools, including, but not limited to: customer service; technical support; QR codes; chatbots; search tools; employee recruitment tools; clinician profiles; location listings; digital signage; and subscriptions, reminders, alerts and marketing messages using direct mail, email and SMS (text messages).
“You” or “Your” — You, regardless of being an individual, a group or an organization, and regardless of whether you are using Endeavor Health Services on behalf of yourself or someone else.
“Your information” — Collectively refers to an individual’s personally identifiable information (“PII”), and/or protected health information (“PHI”).
Personally identifiable information (“PII”) — An individual's first name or first initial and last name in combination with any one or more of the following data elements: Social Security number; driver's license or state identification card number; account number, credit card number or account information related to a financial account; medical information; health insurance information; and/or unique biometric data generated; OR, a username or email address, in combination with information that would permit access to an online account. See the Illinois Personal Information Protection Act for more information.
Protected health information (“PHI”) — Any information regarding an individual's medical history, mental or physical condition, or medical treatment or diagnosis by a healthcare professional, including such information provided to a website or mobile application.

About your protected health information (“PHI”)

Unless otherwise specified, please note that this Privacy Statement only applies to personal information you provide through your use of the Endeavor Health Services. As otherwise not legally prohibited, Endeavor Health and third-parties may collect personally identifiable information (“PII”) that you choose to provide to the Endeavor Health Services. When necessary, you will be prompted to accept additional terms and conditions when using Endeavor Health Patient Portals via a web browser and/or mobile app (see “Endeavor Health Patient Portals and what we collect”).

This Privacy Statement does not apply to any protected health information (“PHI”) gathered via the Endeavor Health Services. Rather, Endeavor Health’s Notice of Privacy Practices will govern Endeavor Health’s use and disclosure of your PHI. The Notice of Privacy Practices describes how information about Endeavor Health’s patients may be used and disclosed. Please review it carefully.

For more information, including information about your rights under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (collectively, “HIPAA”), please review Endeavor Health’s Notice of Privacy Practices.

Acceptance of the Endeavor Health Privacy Statement

By accessing the Endeavor Health Services and/or using any of the features or services provided on the Endeavor Health Services, you signify your acceptance of the Endeavor Health Privacy Statement and consent to the information collection and use practices described herein. If you do not consent or otherwise do not agree with the Endeavor Health Privacy Statement, please do not use or access the Endeavor Health Services.

Information we collect & how it is collected

Endeavor Health may collect your PII through the Endeavor Health Services. We may collect PII actively (i.e., by collecting information you voluntarily submit to Endeavor Health) and passively (i.e., by collecting information automatically gathered from your computer).

Personally identifiable information (“PII”)

In some instances, Endeavor Health, through the Endeavor Health Services, and/or a third-party vendor or other third parties may collect PII (including but not limited to email address, Internet Protocol (IP) addresses, name, phone number, and other information as defined under the Illinois Personal Information Protection Act). This information may be collected when accessing the Endeavor Health Services or when volunteered by the user (e.g., when you create a patient portal account, when you schedule an appointment, when you submit a web form or when you answering a question prompt posed by a chatbot tool). If you send Endeavor Health an email request or participate in our forums, Endeavor Health may store that information as well, including any PII included in a post, message or email, as well as any PII included in any attachments you may include.

When applicable, you may also use the Endeavor Health Services to pay a bill, buy a gift from a gift shop or make a donation (collectively, “Transactions”). To process a Transaction, Endeavor Health or one of our third-party vendors (“Payment Processors”) may collect PII, such as your banking information, like your name, billing address, credit or debit card number, email address, telephone number and/or email address. We or Endeavor Health’s Payment Processors will process your credit card payment using established encryption techniques that protect your information.

If a Payment Processor processes your Transaction, Endeavor Health encourages you to read such Payment Processors’ privacy statement or policy and terms of use before entering any PII. Endeavor Health may not receive complete payment information for a third-party vendor-processed Transaction, but Endeavor Health’s Payment Processor will provide Endeavor Health with some PII upon completion of the Transaction, such as your name, billing address, email address, telephone number, amount paid or donated, how you heard about Endeavor Health, and the last four digits of your credit card number, the latter of which Endeavor Health uses for verification and/or customer service purposes.

Also, when you visit the Endeavor Health Services, Endeavor Health’s servers may automatically record certain information about the device (e.g., a computer, tablet, or mobile device) you used to access the Endeavor Health Services. If a third-party vendor collects your PII, such vendor may share and/or use all or part of your PII with Endeavor Health or another third party. In addition to other uses and disclosures in the Endeavor Health Privacy Statement, Endeavor Health may also disclose PII, as required or permitted by law.

Platform information

When you visit Endeavor Health Services, Endeavor Health’s servers automatically record certain information about the device (e.g., a computer or mobile device) you used to access the Endeavor Health Services. Such information, often referred to as “log file” information, may include your device’s IP (“Internet Protocol”) address, operating system name and version, and browser name and version, as well as the referring URL, number of times your device has accessed the Endeavor Health Services, pages of the Endeavor Health Services you viewed, links you clicked, and other information about your visit to and use of the Endeavor Health Services. With your consent, Endeavor Health’s servers may also collect information about the precise location of your device. If you do not wish to provide location data, please consult your web browser and/or device support documentation for information on how to disable these features.

If you access the Endeavor Health Services from a smartphone or tablet, Endeavor Health’s servers may also collect information specific to your device. This information may include your mobile device’s hardware model, unique device identifier(s) and mobile network information. With your consent, Endeavor Health may also access and collect information from certain native applications on your device, such as your device’s camera, photo album, microphone, phonebook or calendar applications. If you do not wish to provide access to native applications, please consult your web browser and/or device’s support documentation for information on how to disable these features.

Text messaging communications

When you sign up to receive text messages from us via the Endeavor Health Services, we collect your name and phone number, information about your browser, information about the page you signed up on, and any other additional information you may provide to us. For more information on text messages, including instructions on how to opt out, please read our Terms of Use.

Employment, fellowship and grant applications

If you contact us in relation to your employment, fellowship, or grant prospects, we will use your personal information to consider you for current and future employment, fellowship, or grant opportunities and to contact you with respect to such opportunities at Endeavor Health that you have expressed an interest in. In connection with your application we may collect personal identifiers, such as your name, postal address, email address, or other similar identifiers; and professional or employment-related information about you, such as current or past job history or performance evaluations.

Endeavor Health Patient Portals, Other HH Mobile Apps & what information we collect

This section titled “Endeavor Health Patient Portals, Other HH Mobile Apps & what information we collect” only applies to an individual’s access and use of the Endeavor Health Patient Portals and/or Other HH Mobile Apps. Other HH Mobile Apps may be created and updated by third-parties and subject to such third-parties’ own privacy policy/statement and terms and conditions of use that you can find on an app store or on that third-parties’ website. Please review this information carefully before using Other HH Mobile Apps.

To provide patients and visitors with a useful and personalized patient experience, the Endeavor Health Patient Portals may require access to additional features and/or information stored on your web browser or smartphone. The following describes the ways Endeavor Health may access, collect, use and share your PII and/or PHI:

Location: Endeavor Health Patient Portals do not collect or store your location data or share it with third parties without your consent. Location data can be used by mobile app features, such as sending notifications to Endeavor Health’s registration staff when you've arrived for an appointment or suggesting healthcare providers or locations that are nearby.
Endeavor Health Patient Portals may offer location-based check-in for in-person appointments or allow you to find healthcare providers near you. The first time you try to use any features that use your location, you will be asked for your consent to share your device’s location information within the app. Your location information will be used only if you give your consent, and your consent can always be revoked by changing your privacy permissions. You do not have to provide consent if you do not want to allow the Endeavor Health Patient Portals to use your location.
Camera and photo library: Endeavor Health Patient Portals may use your device’s camera feature, or require access to your photo library, to let you take and/or share photos or to capture and stream video for telehealth visits. If you choose to allow the Endeavor Health Patient Portals to interact with your photo library, you can upload photos to personalize an account or provide health record attachments and these photos may be accessed by the healthcare providers.
Microphone: If you choose to allow the Endeavor Health Patient Portals to use your device’s microphone, you can use your microphone to navigate the Endeavor Health Patient Portals and/or can be used to capture audio for telehealth visits. The Endeavor Health Patient Portals do not store your audio data.
Storage: Endeavor Health Patient Portals may store files you upload from your device's storage to your medical record. The Endeavor Health Patient Portals may access your device's storage to read and write files you select in the application. These files may be used as file attachments that are sent to your healthcare provider or they may be created from file attachments sent to you from your healthcare provider.
Bluetooth: Endeavor Health Patient Portals may access your device's Bluetooth feature. Bluetooth may be used to detect other devices nearby that can help with your appointment arrival. The Endeavor Health Patient Portals do not store your Bluetooth data.
Phone: Endeavor Health Patient Portals may use your smartphone to call phone numbers selected by you in the application. The Endeavor Health Patient Portals will not collect, store or share your call history or any other call data.
Telehealth: Endeavor Health Patient Portals may allow you to conduct a telehealth appointment with your healthcare providers, such as a video visit or a phone appointment that may be subject to additional terms and conditions for you to review and accept. Always confirm with your provider if they provide telehealth services. The Endeavor Health Patient Portals only provide the technical capabilities for telehealth appointments to happen. Endeavor Health Patient Portals do not capture any health information about you that is discussed or exchanged with your provider during live telehealth appointment sessions.

In addition to giving you a notice of technology changes, if subsequent versions of Endeavor Health Patient Portals collect new types of PHI and/or PII, you may be notified by a pop-up message of these changes on Endeavor Health Patient Portals with an explanation of why new data is being collected.

Endeavor Health Patient Portals may interact with your PII or PHI to provide you with certain features, such as video visits or mobile appointment check-in. Please refer to Endeavor Health’s Notice of Privacy Practices to learn how Endeavor Health handles your PHI. The first time you try to use any of these features, you will be asked for your consent within the Patient Portal and you may only use a feature if you give your consent. You do not have to provide consent if you do not want to allow the Endeavor Health Patient Portals to interact with your data as requested.

The Endeavor Health Patient Portals are developed by or in conjunction with Epic Systems Corporation; please refer to the Epic Mobile Application Privacy Policy for Patients for more detailed information about the limited ways Epic may interact with your information to make your use of the Endeavor Health Patient Portals possible.

The Endeavor Health Patient Portals were not created specifically for the COVID-19 pandemic. They existed before the COVID-19 pandemic to allow you to access your health information on file. We allow you to access COVID-19-related vaccination information, laboratory test results and documents with illness-related information using the Endeavor Health Patient Portals. You may choose if or how you want to access, display or use such information, just like you can make those decisions about health information relating to other conditions, services, tests or vaccinations.

Privacy of children

Because Endeavor Health is committed to protecting the privacy of children, the Endeavor Health Services are not intended for use by or directed at children under the age of 13. We do not knowingly nor intentionally collect PII from anyone under 13 years of age via Endeavor Health Services. We encourage parents and guardians to take an active role in their children’s online and mobile activities and interests. If we are made aware that we have received such information, or any information in violation of our policy, we will use reasonable efforts to locate and remove that information from our records.

Cookies and other tracking technologies

Endeavor Health, and some of Endeavor Health’s third-party vendors, may use cookies to help Endeavor Health count how you access, browse, and use the Endeavor Health Services (e.g., counting the number of times you and other users visit a webpage or access a feature), thereby providing Endeavor Health with information that can be used to improve the Endeavor Health Services.

Cookies are small data files locally stored on the device used to access the Endeavor Health Services (e.g., computer, smartphone, tablet). Web beacons are electronic images that Endeavor Health or Endeavor Health’s third-party vendors may use in the Endeavor Health Services or in emails to deliver cookies, count visits, understand Endeavor Health Site usage, and/or evaluate the effectiveness of marketing campaigns.

Cookies have different durations and functions. Regarding the duration, there are two types: session cookies and persistent cookies. A session cookie only lasts for the specific duration of your visit to a website and is deleted when you close your browser. A persistent cookie remains in your device's data storage until it is deleted or it expires, which occurs thirteen months after being generated.

As for functionality, there are also two types of cookies: essential cookies and non-essential cookies. An essential cookie is a cookie that is either (i) used solely to carry out or facilitate a transmission to server hosting a website, or (ii) necessary to provide an online service you requested. A non-essential cookie, as the name suggests, is a cookie that may enhance the performance or functionality of a website, be required to access a portion of a website or be used for analytics or marketing (e.g., targeted advertising).

Endeavor Health and Endeavor Health’s third-party vendors may use one or more of the following types of cookies to facilitate access to various features of the Endeavor Health Services.

Essential Cookies are required to enable the basic features of this site, such as providing secure log-in.
Functional Cookies allows Endeavor Health to provide personalized features and/or enhance the performance of an experience, such saving a location, chat services and user preferences.
Analytics Cookies are used to understand how visitors interact with the website. These cookies help provide information on metrics such as the number of visitors, bounce rate, traffic source, etc.
Performance Cookies are used to understand and analyze the key performance indexes of the website which helps in delivering a better user experience for the visitors.
Marketing Cookies are used to provide visitors with customized advertisement based on previous actions, such as a website visit or viewing a marketing email, and used to analyze the effectiveness of an advertising campaign.
Other Cookies are cookies that are being analyzed and may have not been classified into a category.

Endeavor Health may use third-party services, such as Google Analytics, that provide Endeavor Health with usage analytics. Such analytics may help Endeavor Health track usage of the Endeavor Health Services, identify popular features, provide advertising, share content specific to your use of the Endeavor Health Services, and better understand the online activity of Endeavor Health’s patients and visitors of the Endeavor Health Services.

Cookies may allow Google to recognize a user when a user visits the Services and when the user visits other websites. Google uses the information it collects from the Services and other websites to share with us and other website operators’ information about users including age range, gender, geographic regions, general interests, and details about devices used to visit websites and purchase items. We do not link information we receive from Google with any of your PII. For more information regarding Google’s use of cookies, and collection and use of information, see the Google Privacy Policy (available at https://policies.google.com/privacy?hl=en). If you would like to opt out of Google Analytics tracking, please visit the Google Analytics Opt-out Browser Add-on (available at https://tools.google.com/dlpage/gaoptout).

You may still use the Endeavor Health Services if you reject, block and/or delete Endeavor Health’s own or third-party vendors’ cookies, but please note that you may not be able to access some areas or features of the Endeavor Health Services. How we use & protect your information

The following information below provides you with other privacy information related to the Endeavor Health Services and describes other ways how Endeavor Health, through its Endeavor Health Services, and third parties may store, use and disclose your PII.

Unless legally prohibited, Endeavor Health, through the Endeavor Health Services, may use, store, disclose and/or share PII as set forth in this Privacy Statement. Endeavor Health may also use the PII you provide to the Endeavor Health Services in the following way:

to provide you with services you request through the Endeavor Health Services and maintain your account with us;
to register you to use the Endeavor Health Services;
to contact you about your use of the Endeavor Health Services or about Endeavor Health’s programs, services, products, activities, special events or other news that may be of interest to you;
to provide, maintain or improve the Endeavor Health Services;
to provide you with customer service;
to process financial transactions, such bill payments, gift purchase or donations;
to detect, investigate and prevent fraudulent credit card transactions;
for general security monitoring;
to publish in annual reports or other public documents that include donors’ names and information relating to the amount of their respective donations (e.g., by indicating a donation level or tier)
to schedule appointments or place you in contact with one of Endeavor Health’s healthcare providers;
to provide medical advice from your providers through telemedicine services or through Endeavor Health Services;
to manage and host telemedicine services;

We may use Your email address to deliver newsletters, as well as for other promotional purposes (e.g., new service offerings, upcoming events or new location opening). We may send email messages that contains code that enables Endeavor Health’s database to track your usage of the emails; such usage includes, but is not limited to, whether the email was opened and what links (if any) were clicked.

If you do not want to receive informational communications from Endeavor Health, such as email communications informing you of programs, services or products, please see “Choices you have regarding the use of your PII” regarding your options to opt-out of receiving such communications.

Communication you send to Endeavor Health via email or form submission on Endeavor Health Services may be shared with the appropriate Endeavor Health staff member and/or third-party vendors. Email does not provide a completely secure and confidential means of communication. Please do not send any PHI via unsecured email. How we process your PII

Endeavor Health is committed to processing your PII in accordance with applicable data privacy laws and with transparency and fairness. We will only process your PII if:

We have your consent to process and use your PII;
We need to process your PII in order to complete a transaction with you or to fulfill a contractual obligation owed to you;
We have a legitimate business purpose for processing your PII, such as to improve and develop the Endeavor Health Services or services offered on or through the Endeavor Health Services, to prevent fraud or provide general security monitoring, or to improve user experience; or
We are required to process your PII in order to comply with an applicable law or regulation.

How we share your PII with third parties

Endeavor Health will not sell, share, trade, or rent any PII to others in ways different from what is disclosed in the Endeavor Health Privacy Statement without first obtaining your authorization and consent.

In addition to other uses of PII as described in the Privacy Statement, Endeavor Health may allow third-party vendors and/or other third parties to collect, use and disclose your PII.

We may disclose your PII you provide Endeavor Health to third parties (e.g., independent contractors, volunteers, vendors, service providers or consultants) who are engaged by or working with Endeavor Health and who need access to such information to carry out their programs or services. Such services include but are not limited to billing, payment processing, providing medical advice for telemedicine services, management and hosting of telemedicine services, customer service, email deployment, business analytics, talent acquisition services, marketing (i.e., advertising, attribution, deep-linking, direct mail, mobile marketing, optimization, performance monitoring, hosting, and data processing) (collectively “Specialized Services”). These third-party vendors and service providers may not use your information for purposes other than those related to the Specialized Services they are providing to Endeavor Health.

We may also provide aggregate information (i.e., information collected from you that does not allow you to be personally identified or contacted) to third parties without your authorization, such as information about your access and use of the Endeavor Health Services.

We may also disclose Your Information your PII that Endeavor Health has collected if required to do so by law or if, in Endeavor Health’s good faith judgment, such action is reasonably necessary to comply with a legal process, to provide general security monitoring, to respond to any claims or to protect the rights of Endeavor Health and/or Endeavor Health’s patients, physicians, healthcare providers, partners, vendors and/or its constituents and the public.

Please note that the Endeavor Health Services may contain links to third-party websites. Please be aware that Endeavor Health is not responsible for any privacy policy or statement of such third-party websites. We may use third-party ad servers or ad networks to serve advertisements. Advertisements are not on or within Endeavor Health’s site, they are on external websites and search engines. These advertisements link to the Endeavor Health Services. Third-party ad servers or ad networks may use other technologies such as cookies, scripting language or clear gifs to measure the effectiveness of their advertisements and to personalize the advertising content you see. Within Endeavor Health’s site, Endeavor Health may provide a hyperlink to a third-party vendor’s website for the purpose of that third-party vendor performing Specialized Services. While visiting their website, these third-party vendors may use their own cookies and tracking software and Endeavor Health recommends you review their website terms of use and privacy policy or statement. We encourage users of the Endeavor Health Services to be aware when they leave the Endeavor Health Services and to read the privacy policies or statements of every website that collects PII. Endeavor Health is not responsible for the terms of use, security, and privacy policy that these third-party vendors employ while you visit their website.

Similarly, if you entered one of the Endeavor Health Services through another website, Endeavor Health is not responsible for the privacy policy of that site, and you should review the privacy policy of the originating website before providing any PII to that website. The Endeavor Health Privacy Statement applies solely to information collected by Endeavor Health via the Endeavor Health Services.

How we protect your PII

Protecting your PII is of the utmost importance to Endeavor Health. Thus, We use security measures to protect your information and continuously monitor activity across Endeavor Health’s technology infrastructure. We implement technical and organization security measures, including physical, administrative, and technical safeguards to protect your PII from unauthorized access, use and disclosure.

Once Endeavor Health receives your information, a reasonable effort is made to ensure its security on Endeavor Health’s systems.

How you can access, update & modify your PII

If you are a Endeavor Health patient, you may access, verify, modify, and/or update limited account information using a Endeavor Health Patient Portal. If you do not have Endeavor Health Patient Portal account, you can either create an account or you can contact Endeavor Health to verify, correct or modify any personal information.

For PII, you can contact Endeavor Health to have that information updated and/or modified.

To request the deactivation of a patient portal account or to update and/or modify other PII, please contact Endeavor Health. We will make every attempt to update and/or modify your PII as you request, provided that such PII is not necessary to complete any service or transaction and is not otherwise prohibited by law. If you wish to deactivate an account(s) you have with Endeavor Health, please note that Endeavor Health may retain some information from or about you in accordance with applicable law, including for various legal, operational, accounting and auditing purposes.

How to manage cookies

There are several ways to manage cookies. You can configure your browser to refuse all cookies or to indicate when a cookie is being sent. The “Help” feature on most browsers provides information on how to accept cookies, disable cookies or to notify you when receiving a new cookie. Please note, however, that many of the cookies Endeavor Health uses are "strictly necessary" cookies. By blocking or deleting them, you will not be able to access certain features of the Endeavor Health Services.

As described in the Endeavor Health Privacy Statement, third-party advertisers that may collect information from you or about you on the Endeavor Health Services to provide you with more relevant advertising on websites you visit. Such third-party advertisers may provide you with the option to opt out of their services.

Some third-party advertisers participate in the Digital Advertising Alliance Self-Regulatory Program for Online Behavioral Advertising. This program offers a centralized location where users can make choices about the use of their information for online behavioral advertising. To learn more and to make choices about the use of your information for online behavioral advertising on websites, please visit the Digital Advertising Alliance website. Please note that if you opt out of online behavioral advertising using this method, this opt-out will only apply to the specific browser or device from which you opt out.

In addition, some third-party advertisers, advertising technology companies, and service providers that perform advertising-related services for Endeavor Health’s business partners and Endeavor Health may be members of the Network Advertising Initiative (“NAI”), a cooperative of online marketing companies that offers a centralized tool for opting out of interest-based advertising delivered by each of its member companies. If you would like to obtain more information about interest-based advertising and the NAI and make choices about their members’ use of information from or about you on websites, please visit the Network Advertising Initiative website. Patient portal communications preferences

We want to communicate with you on your terms. Endeavor Health Patient Portals can help connect you with your healthcare provider and/or other Endeavor Health staff members (e.g., nurses, pharmacists, HIM etc.…) by sending you messages or notifications to your email, phone and mail regarding appointments, test results and more. You can set communications preferences on how you would like to be contacted by Endeavor Health healthcare. Log in to a Endeavor Health Patient Portal to verify and adjust your patient portal communication preferences.

Opt-out requests

You may not be able to opt out of receiving all communications from Endeavor Health, such as administrative messages, service announcements, messages regarding the terms and conditions of an account or other messages that may contain important information regarding an account to the extent that such information is necessary to complete any services or transactions you have entered into.

Depending on the subscription, you may be able to opt out of receiving optional communications from Endeavor Health, by selecting the "unsubscribe" link from email communications. Depending on the SMS coding, You may be able to opt out of SMS text messages by following the SMS opt out instructions (e.g., replying with “STOP”); please note that this action will permanently opt you out of SMS texts and you will need to manually opt in to restore SMS messages.

If you would prefer that Endeavor Health does not share your PII with Endeavor Health’s business partners or vendors for direct marketing purposes, or to opt-out entirely of all non-required communications, you may send your request to marketing@northshore.org.

There is currently no consensus among industry participants as to what “Do Not Track” means and how to respond to "Do Not Track" browser signals. As such, Endeavor Health do not respond to or honor such signals. Instead, to opt-out of website-based, third-party, interest-based or online behavioral advertising, please refer to the "How to manage cookies" section.

Law applicable to this Privacy Statement

Endeavor Health is headquartered in the State of Illinois, and the Endeavor Health Services are intended for users in the United States. By viewing any content or otherwise accessing the Endeavor Health Services, you consent to the transfer of information to the United States to the extent applicable, and the collection, storage, and processing of information under the laws and regulations of the State of Illinois and the United States.

We may update or modify this Privacy Statement from time to time in response to changing legal, technical or business developments. We will obtain your consent to any material Notice changes if and where this is required by applicable data protection laws.
